After the appendix operation, we had to operate again for ovarian torsion!
Remember last week we brought the report of an orphan who had an acute appendicitis that was operated upon and she needed about 30 thousand naira, about $75, to offset her bills, and a certain young man offered to clear up the bills with 50k? Well, she is hail and hearty now and was discharged yesterday.
She had the appendicectomy and few days afterwards started complaining of this constricting pain that was located in her left lower abdomen, it was severe she would writhe on the ground in pain, go naked and cry all day. She was 6weeks pregnant. She wasn't bleeding and there was no trauma to that part of the abdomen.
The RDI team was very worried. We requested for an USS and it came back the same as the preoperative one that showed a left ovarian cyst. Well, we had to make the diagnosis of an ovarian torsion as a result of the Left luteal cyst and after engaging my senior colleagues and discussing at length, we decided to manage her conservatively until the 16week for the placenta to take over the work of the luteum on the early pregnancy. We did everything but it got to a point where the lady couldn't take it any more, the pain was excruciating. We explained the situation to her and the probability that she may lose the pregnancy. She agreed to the operation, we went in and realized that what we thought was the left ovarian cyst was actually the right that had been contused to the left part and causing all the pain. We carefully removed the cyst and it's capsule from the right ovary, returned it to the right side, and closed up.
48hrs later she was fine. 4 days later she had a miscarriage. We are hopeful that she would be fine in future as both ovaries were preserved.
God has a way with everything. She went through hell with that pregnancy for a reason. She will be compensated I know. The rural doctor will continue to work tirelessly to provide quality care to the rural people who need it the most. Here she is appreciating Mr Francis who paid partly for her bills.
