He is 29years old and presented some time ago with this sudden deepening yellowing of the eyes, chest pain and difficulty breathing. He had been at home taking some herbal concoctions mixed by his family and some pills from the chemist. It only got worse and they presented to the facility after having been convinced to come. We took a detailed history coupled with examinations and came to the conclusion that he had chest syndrome in a sickle cell disease patient with haemolysis. Test results revealed a PCV of 12%, genotype of ss and strongly positive malaria parasite. Sickle cell patients have this ability to be stable at very low PCV. A condition called steady state PCV. In this case, however, this must have gone below that PCV and needed transfusion immediately. We treated him according to the protocol and before long he was stable again but as management progressed, the relatives demanded to leave against medical advice telling us he was better and they had no more money to continue his management. An all too familiar story here. Without a medical doctor and other professional health providers in the rural area, this success wouldn't have been recorded and most probably he wouldn't have survived it.
